What is Service Coordination?

Service coordination is a supportive role designed to help families like yours navigate and access the various services and resources your child may need. As your service coordinator, Jessica Moore is here to:

Assess Needs: Understand your child's developmental needs and listen to your concerns.

Develop Plans: Create individualized family service plans (IFSP) tailored to your child's specific requirements.

Connect Services: Link you with the right services such as speech therapy, occupational therapy, physical therapy, special instruction, and ABA therapy.

Coordinate Providers: Act as a liaison between your family and service providers to ensure cohesive and comprehensive care.

Support Families: Offer ongoing support and guidance, helping you understand and advocate for your child's needs.

Monitor Progress: Regularly review and adjust plans based on your child's development and progress.

Regular Meetings: Meet with you every three months or whenever a new service needs to be added to the IFSP to ensure we are meeting your child's needs.

Atlanta Youth Service's goal is to ensure that your child receives the timely and appropriate interventions needed to support your child's development. We are here to help you every step of the way.
